Gazyva Information

Gazyva (Obinutuzumab) is a monoclonal antibody used to treat certain types of blood cancers. It specifically targets the CD20 protein found on B-lymphocytes, leading to the destruction of these cancer cells. It is commonly used in combination with other medications for more effective treatment.

Uses

  • Chronic Lymphocytic Leukemia (CLL): Used in combination with chlorambucil.
  • Follicular Lymphoma (FL): Used in patients who have relapsed or are refractory to prior treatment.

Mechanism of Action

Obinutuzumab is an anti-CD20 monoclonal antibody. It binds to the CD20 antigen on B-cells, triggering antibody-dependent cellular cytotoxicity (ADCC), complement-dependent cytotoxicity (CDC), and direct cell death, leading to B-cell depletion.

Dosage

Chronic Lymphocytic Leukemia (CLL):

  • Cycle 1: 100 mg on Day 1, 900 mg on Day 2, and 1000 mg on Days 8 and 15.
  • Subsequent cycles (2-6): 1000 mg on Day 1 of each cycle (every 28 days).

Follicular Lymphoma (FL):

  • Induction: 1000 mg on Days 1, 8, and 15 of Cycle 1, then 1000 mg on Day 1 of subsequent cycles.
  • Maintenance: 1000 mg every 2 months for up to 2 years.

Side Effects

Common Side Effects:

  • Fatigue, nausea, diarrhea, and fever.
  • Infusion-related reactions such as chills and low blood pressure.

Serious Side Effects:

  • Tumor lysis syndrome.
  • Infections due to weakened immune response.
  • Hepatitis B virus reactivation.

Precautions

  • Premedication with antihistamines, antipyretics, and corticosteroids is required to minimize infusion-related reactions.
  • Monitor patients for signs of infection or hepatitis B reactivation.
  • Live vaccines should be avoided during treatment.

Onset and Duration of Action

  • Onset: The therapeutic effect can be seen after the first infusion.
  • Duration: B-cell depletion may persist for several months post-treatment.

Contraindications

  • Known hypersensitivity to Obinutuzumab or any component of the formulation.
  • Severe infection or active hepatitis B infection.

Storage

  • Store refrigerated at 2°C to 8°C.
  • Do not freeze or shake. Protect from light.
