Clinical Background:
Human leukocyte antigen B27 (HLA-B27) is a major histocompatibility complex class 1 molecule that is strongly associated with the
disease ankylosing spondylitis. Testing for HLA-B27 is of diagnostic value because more than 90% of patients with ankylosing
spondylitis possess B27 allele, but only 1% of people with HLA-B27 develop the disease.
HLA B27 is also known for its associations with other diseases (like Reiter’s disease, Acute anterior uveitis, Psoriatic arthritis,
Juvenile idiopathic arthritis).
Limitations:
• HLA B7 can cross react with anti-HLA B27 antibody giving false positive test result. This can be confirmed by alternate
method discriminating B7 & B27 or by ancillary molecular techniques.
• Since the assay is cut-off based analysis by Flow cytometric expression of HLA B27 antigen, any result +/- 10% of cut-off is
considered as Borderline, which can be confirmed by Molecular assays like Microarray.
• HLA B27 should only be used as an adjunct to clinical diagnosis since it only gives predisposition for the development of
disease.
Recommendation:
All HLA B27 Positive, Borderline Positive & Borderline Negative results are recommended to be confirmed by Microarray
Biochip Technology (Available at Metropolis) for determination of Disease Associated HLA B27 Alleles in humans. Since all
the HLA B27 alleles are not associated with Ankylosing Spondylitis (e.g. B*27:06 & B*27:09), HLA B27 testing by Flow
cytometry may give positive results due to expression of Non-Disease associated alleles as well.
